Joining Scooper Activities
The Activities Departments at Stagebarn MS, Sturgis Williams MS, and Sturgis Brown HS invites students to participate in the many activities that Meade School District has to offer. Our activities are integral to the educational process and strive to develop lifelong skills. We want our students to be committed, respectful, strong, and resilient while they represent our school with the highest degree of integrity. Any student wishing to join an activity must first register to participate.
Things to do prior to registering.
Schedule a physical with your primary care physician or you can also receive a free physical screening at Monument Health. You must schedule an appointment at the clinic.
Print both Health History Form and Preparticipation Physical Exam Form prior to the physical. Take both with you to the physical exam. You will need to fill out the Health History Form prior to the physical exam. Both forms require a signature by the parent/guardian and student. There also needs to be a doctor's signature on the Preparticipation Physical Exam Form. It is important that you hold on to both the forms. These will need to be uploaded during registration.
Read through the following forms before you register. Your electronic signature during registration will verify that you have read and understand each of these forms.
If you are a student involved in alternative instruction - Home School Student - you must fill out the Alternative Instruction Form and have the previous semester's transcript available to upload.
All students involved in activities must read and agree to abide by the Student Activities Manual. This manual gives information about training rules, transportation rules, eligibility rules, and other general rules of participation.
If interested in purchasing the Student Accident Insurance plan, you can go to 1st Agency for a brochure and to purchase at a very nominal cost.
Once all above items have been completed or read through, you can prepare for registration by reading through the Guidelines for Registration. Keep this open and follow along while you go to the appropriate registration site below.
In Case of Injury
As a preventative measure, students are encouraged to participate in the Student Accident Insurance Program
In the event of an injury, participants ought to complete the Student Accident Report Form.
